- Links to all necessary forms can be found here. These forms will also be attached to the emails for each day of the convention to which you register.

- Please note that all forms will need signed and either mailed or emailed to our CE Administrator no later than June 15, 2021. Please mail them to: 2405 Maryland Ave., Metairie, LA 70003 or email them to: LSHAContEd@yahoo.com.
