Audiology & Supervision Track- Member – $25.00
This Registration type is for LSHA members who wish to attend the Audiology track session, scheduled for Thursday, June 4th.
Audiology & Supervision Track- Non-LSHA Members – $60.00
This registration type is for non-LSHA members who are interested in attending the Audiology track, scheduled for Thursday, June 4th.
Audiology & Supervision Track- Student – $15.00
This registration is for LSHA student members who wish to attend the Audiology track, scheduled for Thursday, June 4th.
Medical Track- LSHA Members – $25.00
This registration type is for LSHA members who wish to attend the Medical track, scheduled for Saturday, June 6th.
Medical Track- non-LSHA Members – $60.00

Please find the ASHA CEU Participant form here and LSHA tracking sheets here: Thursday, Friday, Saturday. These forms are also attached to the email for each day of the convention for which you are registered.

All forms must be ink-signed and mailed (not emailed) to the CE Administrator by June 16 in order to be processed. Mail to: CE Administrator, 2405 Maryland Ave., Metairie, LA 70003.
